All covered entities at Stanford and at the Veterans' Administration need to be HIPAA compliant.

Consent Forms
If a study protocol involves the use or disclosure of PHI (Protected Health Information), either the consent form must include HIPAA language, or there must be a separate HIPAA Authorization Form in addition to the consent form.
